  - Print it and fill it out
  - Or download & digitally fill it out
  - Don’t forget to sign the form (or have parent/guardian sign it)
- Scan signed file (clear phone images accepted)
- Submit image file below

- Please be sure your file is saved with a name that you mention in your form (i.e. MaggiePieEntry.jpg)
- If your submission is a physical submission (i.e. sculpture, painting) please submit a pic of it here and then see info on where to take the physical submission
